 Original or notarized copy of parents’ marriage certificate
 Original or notarized copy of permanent residence booklet in China or Taiwan
 Documents showing the termination of prior marriage(s) : a divorce decree, death certificate, or annulment document
 Child’s school record ( if child is more than 5 years’ old)
 Proof of the Parent(s)’s residence or physical presence in the United States (School, employment, military records; deeds, mortgages, leases showing residence; Attestations by churches, unions, or other organizations; Social Security quarterly reports; Utility bills; Medical records; Insurance policies; Income tax returns (Form 1040); Passports of original residence country（Affidavits of third parties having knowledge of the residence and physical presence)

 Child’s Photographs from birth to present
 Written agreement to promise supporting child until 18 years old and evidence of supporting child
