Affidavits sworn by persons who are familiar with the events of birth of the petitioner
School records (preferably from the first school) showing the date of admission to the school, the childs date and place of birth, and the names and places of birth of the childs parents.

Different documents should be submitted depending on the relationship involved in the petition:
Child born in wedlock:
The childs birth certificate. If the childs name differs from the name on his or her birth certificate, evidence of the name change must be submitted.
Household Registration
Marriage Certificate of the parents
Divorce decrees, death certificates, or other documentation evidencing the termination of all prior marriages of both parents
Marriage Certificate of a married daughter
Child born out of wedlock (filed by the mother):
The childs birth certificate. If the childs name differs from the name on his or her birth certificate, evidence of the name change must be submitted.
Household Registration
If the mothers present name differs from that appears in the childs birth certificate, evidence of the name change must be submitted.

Evidence that a bona fide parent-child relationship is or has been established while the child is unmarried and under 21 years old, or the child has been legitimated under the law of the childs residence before the childs 18th birthday and the father has legal custody of the child

Marriage Certificate of a married daughter
Stepchild:
The childs birth certificate. If the childs name differs from the name on his or her birth certificate, evidence of the name change must be submitted.
Household Registration
The marriage certificate of the step-father/mother to the natural mother/father
Divorce decrees, death certificates, or other documentation evidencing the termination of all prior marriages of the step-father/mother and natural mother/father
Marriage Certificate of a married daughter
Adoptive child:
The birth certificate of the child. If the childs name differs from the name on his or her birth certificate, evidence of the name change must be submitted.
A certified copy of the adoption decree, showing that adoption was completed before the childs 16th birthday
Household Registration
Evidence that the child has been in the legal custody of the parent for at least two years (whether before or after the adoption), and is physically residing with the parent for a two-year period (whether before or after the adoption)

Affidavit of the teacher (the adoptive parents accompany the child)
Certificate of the doctor (the adoptive parents accompany the child and pay it)

Notarized affidavit from Committee of Lin or Li in Taiwan, showing the cohabitation

Marriage Certificate of the adoptive daughter
Siblings of adoptive child (with at least one same parent):
The birth certificate of adoptive childs siblings. If the childs name differs from the name on his or her birth certificate, evidence of the name change must be submitted.
A certified copy of the adoption decree, showing that adoption was completed before the childs 18th birthday and by the same US citizen couple
Evidence that the child has been in the legal custody of the parent for at least two years (whether before or after the adoption), and is physically residing with the parent for a two-year period (whether before or after the adoption)
