Secondary Documentation
Baptismal certificate, with the seal of church, showing the date and place of birth in the United States and the date of baptism.
Affidavits sworn by persons who are familiar with the events of birth of the petitioner
School records (preferably from the first school) showing the date of admission to the school, the childs date and place of birth, and the names and places of birth of the childs parents.

Proof of the relationship
Different documents should be submitted depending on the relationship involved in the petition:
Siblings with same parents
Birth Certificate of the siblings
Household Registration
Marriage Certificate of the parents
A copy of any divorce decrees, death certificates, or annulment decrees that establish the termination of any prior marriages entered into by either of the parents

Siblings with the same mother

Marriage Certificate of a married sister

Siblings with the same father

Marriage certificate of the applicants natural father with each mother
Proof of termination of the prior marriages of the father and each mother, such as divorce decrees, death certificates, or other documentation

Siblings born out of wedlock
The childs birth certificate. If the childs name differs from the name on his or her birth certificate, evidence of the name change must be submitted.
Household Registration
Evidence that a bona fide parent-child relationship is or has been established while the child is unmarried and under 21 years old, or the child has been legitimated under the law of the childs residence before the childs 18th birthday and the father has legal custody of the child

Medical or insurance records
