  Evidence which proved your family relationship (including marriage certificate, divorce adjudgement, birth certificate and household certificate )
  Envelope with the address and stamp

  Baptism certificate

  The pledge about beneficiary?ˉs birth
 According to Act 245(i)?:
  The evidences prove you are in U.S: form I-94 and I-862 (if you applied after January 1, 1998 and before December 21, 2000, you should )
  Government documents, including driving card, identify card, state ID card, hospital certificate, public school report card, income tax return
  Non-government documents: school record, car leasing record, water rate, electrical tab, credit card tab, etc
