1. A valid unexpired passport (if a required passport is not valid when you file Form I-539 application, submit an explanation);

2. Original or copy (front and back) of Form I-94 of each person included in your application;

3. Company letter or a written statement explaining in detail:

1) The reason for the extension request (more business consultations, continuing medical reasons, extended family visit);

2) Why your extended stay would be temporary, including what arrangements you have made to depart from the United States; and

3) Any effect the extended stay may have on your foreign employment or residency;

4. Evidence of financial

1) Bank Account, or bank manager's letter;

2) Pay statement showing regular monthly salary & bank books with regular history of deposits and withdrawals;

3) Employment verification or pension book;

4) Ownership of real statement;
